New Account Application
Thank you for your interest in joining our valued family of
customers. If you would like to apply for an account with
NutraOrigin please read the following New Account application
instructions:
- Please completely fill out the PDF New
Accounts Application form (you can type into the form). Incomplete
applications will be delayed.
- Please print, sign and return a copy of the
New Account Application form to NutraOrigin by fax, scan/e-mail, or
regular mail. Fax: 516-352-3797 or e-mail to customerservice@nutraorigin.com
- Please return the following along with your
application:
- Retail stores and on-line retailers inside New York
State will need a reseller number as well as a New York
State Resale Certificate (available on website).
- Retailers and on-line retailers outside New York State need to
provide a Federal Tax ID and a State Sales/Use Tax
Permit/Certificate or a City/Town/County Business License.
- Practitioners/Physicians/Clinics need to
provide a valid Practitioner or Physician license.
- All Retail stores, On-line Retailers, Practitioners,
Hospitals/Clinics must fill out a W-9 and return it with their
application.
- Approved accounts will be set up once the first order is
placed.
